FORM AND PRESENTATION :
White micro-crystalline powder for IM injectable preparation. Sealed vial of 12ml.
COMPOSITION FOR ONE VIAL :
Benzathine benzylpenicillin (INN) 2.400.000 IU.
EXCIPIENTS WITH KNOWN EFFECT :
None.
PROPERTIES :
Bactericidal antibiotic of betalactamin family, group of penicillin G, giving a long lasting effect (2 to 3 weeks) with
one injection.
Active on Streptococcus, Corynebacterium diphteriae, Meningococcus, Pneumococcus, Gonococcus, Clostridium,
Treponema.
INDICATIONS :
Treatment of the syphilis and the pian (Treponemas), prevent of relapse in acute joint rheumatism.
DOSAGE :
Solvent : WFI 8ml.
Once made up, the suspension must be used immediately.
IM injection only, which have to be deep.
Treponema treatment :
2,4 M.IU each 8 days.
Prevent of relapse in accute joint rheumatism :
Adult : 2,4 M.IU each 15 days.
Children : 600.000 IU to 1,2 M.IU each 15 days.
CONTRAINDICATIONS :
Allergy to penicillin and cephalosporin.
INTERACTIONS :
Avoid association with methotrexate.
In the event of INR imbalance :
Many cases of increased activity of oral anticoagulants have been reported in patients receiving antibiotics. The
risk factors seem to be any marked infection or inflammation, as well as the patient’s age and general condition.
In such circumstances, it is difficult to distinguish whether it is the infection or its treatment that causes the INR imbalance. However, some classes of antibiotics are more involved than others : this is especially the case for fluoroquinolones, macrolides, tetracyclines, cotrimoxazole and some cephalosporins.
PRECAUTIONS AND WARNINGS :
- Do not inject in intravenous way.
- Use when pregnant is possible.
- Avoid during breastfeeding.
- If high doses of penicillin are prescribed, special precautions must be taken due to a risk of nephrotoxicity.
The intrathecal route should be avoided.
- Precautions must be taken when administering high doses of sodium and potassium to patients with kidney
or heart impairment, and high doses of benzylpenicillin potassium should be used with caution for patients treated with drugs containing potassium, or potassium sparing diuretics.
- Kidney function and blood tests should be carried out in the event of prolonged treatment or use of high doses.
- Caution is recommended during treatment of spirochete infections, particularly syphilis, as a Jarisch-Herxheimer reaction is possible.
- Treatment should be avoided in the event of potential allergy to penicillin.
